A consulting room runs on paper until the day someone needs last year’s notes and they are in a folder in another room. INOTDA replaces that with one record per patient: identity and contact details, the intake questionnaire, the clinical history, and every consultation since.

The intake is the part that had to be right, because it is what the doctor fills in while a patient is sitting there. It follows the examination rather than a database schema — reason for the consultation, how long the symptoms have lasted, pain at rest, posture changes, dorsal and lumbar spine, current medication, family history — and it ends where the doctor actually ends, on comments and the treatment plan.

Everything is in Spanish, because the practice is. Each user signs in under their own name and can change their own password, and the whole patient list exports to Excel or a backup file, so the clinic’s records are never locked inside somebody else’s software.
